Do Life Source counselors/therapists provide online (e-mail) or telephone counseling for persons who live outside of our service area?
We are not able to provide counseling or advice via e-mail, telephone or by correspondence. Our counseling is done in person only. We are located in Roseville, California about 15 miles northeast of Sacramento. For those persons viewing our website who live outside our area, we recommend that you speak with your pastor, priest or rabbi regarding your counseling concerns. You may also contact your physician regarding a referral to a counselor in your area.

Do you have evening appointments? What about weekends?
Yes, all of our therapists/counselors offer evening appointments-schedules vary (Monday, Tuesday, Wednesday and Thursday nights -- the last appointment is typically 8:15 p.m.). We do not offer weekend appointments.

What if I do not "connect" with the counselor/therapist?
If at any point in your therapy you feel that your therapist isn't the right fit for you, please discuss this with your therapist. The therapist will assist you in transferring to another therapist on our staff. Our goal is to provide the best possible treatment. Other referrals will be given if needed.

Will my sessions be confidential?
All information disclosed within sessions and the written records pertaining to those sessions are confidential and may not be revealed to anyone without your written permission, except where disclosure is required by law. Some of the circumstances where disclosure is required by the law are: where there is a reasonable suspicion of child, dependent or elder abuse or neglect; where a patient presents a danger to self, to others, to property, or is gravely disabled. In keeping with state and federal legal and ethical mandates, the "Office Policies & General Information" section of the Life Source intake form provides more detailed information about confidentiality.

Will I need to commit to a standard number of sessions?
The number and frequency of sessions will depend on the issues, complexity and the treatment plan goals. After your first appointment, your counselor/therapist will suggest a treatment plan.

What is the cost for counseling and how long is each session?
Life Source, Inc. therapists have varying fee schedules based on years of experience, licensure, and/or degree level. Our office staff will provide information about the session fees. The intake session is about 60 minutes (CPT Code 90801). After the first session, the standard session is about 45 minutes (CPT Code 90806). The 60 minute session is a higher fee than the 45 minute session.

Is the session fee the same if more than one person is in the session?
Yes, the session fee remains the same for one person or a family.
|

What is your cancellation policy?
Since scheduling of an appointment involves the reservation of time specifically for you, a minimum of 48 hours (2 days) notice is required for re-scheduling or canceling an appointment. The full fee will be charged for sessions missed without such notification. We will do our best to reach someone on the waiting list to fill your reserved time and, if successful, then you will not be charged. Insurance carriers, Victims of Crime, etc. will not pay for missed appointments or late cancellations.

What if an appointment is made for conjoint counseling, but one person cannot come, should the other person come in?
Yes, it can be an opportune time to work on individual issues that affect the relationship.

Do you bill insurance?
We do not bill insurance, although we will provide you with a computer generated "Insurance Claim Form" for you to mail to your insurance company for reimbursement.
You will need to request a claim form at each visit so you can mail the claim to the insurance company.
We recommend that you call your insurance carrier regarding your mental health care benefits. Please feel free to ask our office staff about questions to ask the representative about your coverage.

What forms of payment do you accept?
Payment in full for services rendered is due on the day of the service. We accept cash, ATM cards, all major credit cards, or checks. (No personal checks for Pre-Op Evaluations/Bariatric Surgery).

Does Life Source, Inc. accept Victims of Crime patients?
Yes, we will accept patients covered under the state Victims of Crime Program. Our office staff can provide further information.

Can Life Source therapists write prescriptions?
No, our therapists are not medical doctors and cannot write a prescription. You will need to go to your primary care physician or a psychiatrist.

|
Can Life Source therapists coordinate medications with my physician?
Yes. If a patient needs to be referred for possible medication to treat disorders such as anxiety, depression, or ADD/ADHD, your therapist will assist you in coordinating with your doctor.
